REGISTRATION INFORMATION

Age Requirements:

• Kindergarten - any child turning 5 years old on or before September 1, 2007.
  
  
• First Grade - any child turning 6 years old on or before September 1, 2007 who has successfully completed kindergarten or otherwise meets the criteria for admission or transfer.
  

All students attending school for the first time in Florida (Pre-K thru 12) must present:

1. A Student Health Examination (HRS-H Form 3040) including proof of a tuberculin skin test, reading of test, and appropriate follow-up.
2. A Certificate of Immunization (HRS Form 680)
3. Proof of Age
4. Verification of Residence
5. Disclosure at Time of Registration (FM-5740ESH)
